    TKA(人工膝関節全置換術)術後リハビリテーション目的は、VTE(静脈血栓塞栓症)予防、筋力訓練・歩行訓練による早期回復、可動域訓練による拘縮予防・可動域拡大である。周術期の必須アミノ酸投与は、TKA術後ADLの回復を早めるため推奨できる。膝関節可動域は術後約2年間で徐々に改善するため、入院中よりも退院後のリハビリテーションがむしろ重要である。入院中に可動域訓練の方法を十分に教育しておく。退院時に可動域訓練の方法を記した冊子を渡すこと、紹介元の整形外科クリニックと連携して外来リハビリテーションを行うことも有用である。(著者抄録)

    <文献概要>はじめに 人生100年時代といわれる超長寿社会において,疫学的に医療の重要課題は"老化"といわれている.骨粗鬆症は老化に典型的な伴う疾患であり,人類に与える影響は各種がん以上とされる.基礎的臨床的集学的な研究が進み,骨粗鬆症が治療可能な時代となった理由のうち最も中心的な役割を担うのが薬物療法である.一方で,われわれ脊椎外科医が最前線に立って治療にあたるべき骨粗鬆症性椎体骨折(osteoporotic vertebral fracture:OVF)の治療成績は改善されているのであろうか.本稿では,われわれが15年以上にわたって注力してきた臨床OVFのデータとともに現在進行形の研究を紹介し,OVF研究の問題点と今後の展望を私見ながら提案したい.

    This study aimed to investigate the novel iPS cells, which can produce two types of protein; the bone morphogenetic protein-2 (BMP-2) to promote the bone fusion strongly, as well as the herpes simplex virus type 1- thymidine kinase (HSV-TK) to cause apoptotic cell death by ganciclovir administration. This novel iPS cell will allow spine surgeons to perform posterolateral lumbar spine fusion only with injection, instead of with current high invasive surgery. This study had started from 2014. During 2014-2016, we created the DNA vector to deliver the nucleic acids including the gene of BMP-2 and HSV-TK into nuclear of the target cells. Additionally, we confirmed the production of BMP-2 and HSV-TK from the myoblast cell of mouse, which was inrtoduced the genes. Hereafter, we are planning to introduce our vector to iPS cell, after the research to enhance the productivity of protein by the introduced cells.

    The patients with rheumatoid arthritis who have persistent active synovitis only in small joints are found after remission. For those patients, topical treatments such as intraarticular injection of glucocorticoid are indicated. In this phase 2 study, we analyzed the efficacy and safety of injection of methotrexate (MTX) into swollen small joints combined with electroporation (EP) and evaluated by ultrasound at 26 weeks after injection. Three patients received this treatment and 2 received MTX injection with only EP maneuver as control. All patients were allocated in 2 groups randomly.
    No adverse events of skin burn nor pain were found in the treatment group. The degree of synovitis was decreased in 2 of 3 in the treatment group, however those did not change in maneuver group. The number included in this study requires 40 for statistical analysis in the initial protocol, however the population were very small in this time. The study with larger population is needed in the future.

    For peripheral nerve repair, various modifications have been explored. Here, sciatic nerve gaps in mice were reconstructed in the following groups: nerve conduit alone (C group), nerve conduit coated with induced pluripotent stem cell (iPSc)-derived neurospheres (I group), nerve conduit coated with iPSc-derived neurospheres and basic fibroblast growth factor (bFGF)-incorporated gelatin microspheres (I+F group), and autograft (A group). The fastest functional recovery and the greatest axon regeneration occurred in the A group, followed in order by the I+F group, I group, and C group until 12 weeks after reconstruction. Thus, peripheral nerve regeneration using nerve conduits and functional recovery in mice were accelerated by a combination of iPSc-derived neurospheres and a bFGF drug delivery system. The combination of all three fundamental methodologies was essential and useful for peripheral nerve regenerative medicine.
