    <p><b>Background:</b> Cronkhite–Canada syndrome (CCS), chronic enteropathy associated with <i>SLCO2A1</i> gene (CEAS), and intestinal Behçet's disease (BD) are classified as intractable intestinal disorders in Japan. However, the national prevalence of these diseases remains unknown. We performed a nationwide survey to estimate the patient numbers and prevalence rates of these diseases throughout Japan in 2017.</p><p><b>Methods:</b> We conducted a mail-based survey targeting hospitals across Japan to estimate the annual numbers of patients with CCS, CEAS, and intestinal BD in 2017. Using a stratified random sampling method, we selected 2979 hospital departments and asked them to report the number of patients who met specific diagnostic criteria. The total number of patients for each disease was estimated by multiplying the reported numbers by the reciprocal of the sampling rate and response rate. The corresponding prevalence rates per 1,000,000 population were calculated based on the mid-year population of Japan in 2017.</p><p><b>Results:</b> The overall survey response rate was 68.1% (2029 departments). The estimated numbers of patients with CCS, CEAS, and intestinal BD were 473 (95% confidence interval: 357-589), 388 (289-486), and 3139 (2749-3529), respectively; the prevalence rates per 1,000,000 population were 3.7 (male: 4.0; female: 3.5), 3.1 (male: 3.0; female: 3.1), and 24.8 (male: 24.5; female: 25.0), respectively. The male-to-female ratios were 1.10, 0.94, and 0.93 for patients with CCS, CEAS, and intestinal BD, respectively.</p><p><b>Conclusions:</b> Estimates of the national prevalence of CCS, CEAS, and intestinal BD in Japan were generated and found to be higher than those previously reported.</p>

    <文献概要>はじめに 疫学(epidemiology)は,「人集団における健康関連事象(主に疾病)について,頻度・分布と規定要因を明らかにし,その結果を対策に応用する学問」と定義されている.疫学で用いる研究デザインは,おおむね図1のように分類される.すなわち,本項で扱う「記述疫学(descriptive epidemiology)」は,エビデンスレベル(研究結果から得られる根拠の質)がもっとも低い研究デザインに該当する.より質の高い医学研究への興味が高まるなか,特に若手の研究者で,記述疫学研究を軽視する傾向があるかもしれない.しかしながら,記述疫学は,疫学の定義に照らし合わせると「疾病の"頻度・分布"の解明」を担うものである.ある疾病について,患者数などの頻度や,性・年齢などの特性の分布がわからなければ,医療供給体制や行政サービスがどの程度必要か予測できないだけでなく,エビデンスレベルがさらに高い医学研究の計画も立てようがない.すなわち,記述疫学とは,疫学研究デザインの中でも基本の「き」に相当するものであることをまずご理解いただきたい.

    To examine the effectiveness of maternal influenza vaccination on preventing influenza in infants under 6 months of age, we conducted a cohort study among 4,068 infants born before 2013/14 influenza season in Osaka. During 2013/14 season, a total of 85 infants (2%) were diagnosed with influenza and 19 infants (0.5%) were hospitalized with influenza. Maternal influenza vaccination (especially prenatal vaccination) revealed to have a decreasing effect on infants' influenza. Vaccine effectiveness of prenatal vaccination was 60% (95% confidence interval: 13%-82%), whereas that of postpartum vaccination was 48% (-41-81%). Although maternal influenza vaccination was also associated with decreasing infants' influenza-related hospitalization, vaccine effectiveness (70%) could not reach to the statistically significance due to limited number of hospitalized cases.
    Present findings indicated that pregnant women should receive influenza vaccination in order to protect their infants.

    This study aimed to evaluate individual-level factors and area-level indicators in relation to serological testing for hepatitis B or C virus and cancer screening for lung, stomach or colon, with focusing on socioeconomic status. Data from a population-based cross-sectional study involving adults aged 25-64 years, which was conducted in Osaka city, Japan, in 2011, was used for analyses. We found that the effect of area-level socioeconomic indicators was negligible, while the individual-level factors including socioeconomic status were clearly associated with both receiving hepatitis viral testing and cancer screening. Our findings revealed that encouragement at individual-level, not at general population-level, was highly important to increase the response to hepatitis viral testing and cancer screening. A next-generation model to encourage the testing or screening will be needed in consideration of current social trends.
